Please carefully review the following information regarding the enrollment process. Parents and guardians must provide all necessary administrative records before enrollment can be finalized. We encourage you to review the registration forms for a complete list of the conditions and requirements for enrollment.

It is mandatory for students enrolling at T'iis Nazbas Community School to have a parent or guardian accompany them during the enrollment process.

Documents Required

To complete the enrollment process for new students, the following documents and forms are required:

  • Completed Enrollment Packet. All forms in the packet must be completed in full.
  • Immunization Records. Up-to-date immunization records must be provided for each student.
  • Certificate of Indian Blood/Tribal ID. Students applying for admission must demonstrate membership in a federally recognized Tribe and provide a valid CIB and/or Tribal ID.
  • Proof of Age. Please submit a birth certificate or equivalent documentation establishing guardianship or parentage.
  • Social Security Card. Each student must present their Social Security card.
  • Student Attendance/Behavior Log. Students currently under suspension or involved in disciplinary proceedings at another school are required to participate in a documented counseling session before enrollment. This session will review school expectations and address any concerns. 
  • Previous School Report Card. Current transcripts and test data are necessary for enrollment.
  • Individualized Education Plan/Individualized Family Service Plan. If applicable, please provide these documents to ensure appropriate educational support.
